The Billing Process
Exagen Diagnostics will first submit your claim for the AVISE test directly to your insurance company after reporting your AVISE test result to your doctor.
Explanation of Benefits
After processing your claim your insurance company will send you an Explanation of Benefits (EOB). The EOB is not a bill. You should review and contact us at 1-888-452-1522 with any questions.
If your insurance company does not cover all or portion of the cost of the test we will appeal the decision on your behalf. We may require a patient consent form, which we will forward to you.
